Our Mission

The Inner City Family Health Team provides seamless multidisciplinary primary care to homeless and previously homeless individuals in Toronto, with a particular focus on current and former residents of Seaton House. We provide care at our clinic on Queen Street, at Seaton House, and in the community.

If you want help with physical health, mental health, substance use, foot care, and/or community living we can help.

Mission

We address their health, housing, and social care needs while transitioning to community based living.

Values

Respect: We recognize the rights and dignity of all people. We respect their right to have access to good quality healthcare and work with them as partners in their recovery.

Client-Centred and Recovery Orientated

We believe in providing primary care that is recovery orientated, responding to the needs of our clients and taking a harm reduction approach.  We partner with our clients in their recovery, using person centred approaches to reduce the severity of illness and promote well-being.

Housing First

We believe that quick access to safe and affordable housing is a priority for people who are homeless and that other services and supports should follow.

Broad View of Health

We take a person centered approach that includes good health, adequate and affordable housing, good nutrition, a living wage, meaningful activity, social participation, and strong and supportive networks.

Innovation and Learning

We are committed to setting high standards, targets and measures for our performance, continually reviewing and evaluating the quality of the service we provide to ensure we are meeting the needs of clients. We are committed to life-long learning, exploring and developing new, more accessible care pathways for people who are homes and socially and economically marginalized.

Collaboration and Partnership

We believe that we can best meet the needs of those we serve through partnerships, building on community expertise and our relationship with our partners in health, housing and social care.
