Overview
The Inner City Family Health Team provides seamless multidisciplinary primary care to homeless and previously homeless individuals in Toronto, with a particular focus on current and former residents of Seaton House. We address their health, housing, and social care needs while transitioning to community based living.
How do I start getting health care from the Family Health Team?
- Contact our clinic at 69 Queen St East
- Talk to a FHT staff person at Seaton House
- Talk to a Doctor or Psychiatrist at Seaton House or
- Ask a Seaton House staff person to refer you to us.
Services Currently Available at 69 Queen St. East:
Doctor/Nurse Practitioner: Physical and mental health care.
Psychiatry: We have psychiatric time by referral.
Nursing Care: Nursing health care needs, such as chronic illness and counselling.
Chiropody: Foot care. This is a busy clinic, so please make an appointment.
Health Promotion: Life and social skills, healthy living, disease prevention, recreation activities and living in the community.
Counselling: Individual counselling and groups from a recovery and harm reduction orientation.
