Important Notice Regarding COVID-19

We are monitoring the COVID-19 situation closely and following all public health recommendations. As a result we have made changes to ensure the safety of our clients, staff and community members.

If you have any of these symptoms please do not come to the clinic:

  • fever
  • chills
  • muscle aches
  • fatigue
  • shortness of breath
  • sore throat
  • new or worsening cough
  • runny nose
  • loss of taste or smell
  • nausea, vomiting, or diarrhea
  • recently travelled or have been in contact with someone who has tested positive for COVID-19

If you are experiencing any of these symptoms consider getting tested for COVID-19, or call us to speak with a health care provider.

The inner city fht does not do COVID-19 testing – find a COVID-19 assessment centre near you.

  • We kindly ask that only those with an appointment come to the clinic. Please call us if you need to speak with someone or you would like to schedule an appointment — (416) 368-5666.
  • Our office currently has reduced hours – we are open Monday to Friday from 9am to 4pm.
  • Everyone visiting the Inner City Family Health Team is required to wear a mask and answer COVID-19 screening questions before entering the clinic.  You will be provided a new disposable mask after you enter.
  • Only essential support persons are allowed to accompany a client to an appointment (i.e. case worker, translator, care giver, etc.) Non-essential visitors are not allowed to accompany clients to appointments (i.e. friends, family members).
  • Most appointments will be done by telephone, unless your health care provider needs to see you in-person.
  • Some appointments may have to be changed to a phone appointment on short notice. 

Important information about our clinic and services

The Inner City Family Health Team is a primary care clinic – we are currently accepting new clients by community or self-referral for anyone who is experiencing homelessness and needs a family doctor.

(Note: We are not a walk-in clinic and do not provide walk-in services. Our services are open to people that are registered with a doctor at the Inner City Family Health.)

To become a client of Inner City FHT please call (416) 368-5666 to schedule an intake appointment.

Criteria to become a client:

  • Are currently or previously homeless.
  • Currently living in Toronto.
  • Do not have a family doctor elsewhere in the community.
  • Want to be connected to family doctor and wrap around health care.
  • Consented to being referred to ICFHT, or are self-referring.
  • Have a family doctor elsewhere but want to transfer your care to ICFHT.

Note: When making a referral please consider/discuss with the client the location of our clinic and assess if travel/accessibility will be a barrier for them accessing our clinic.

Evening drop-in clinics

Open to anyone experiencing homelessness needing to see a doctor for an immediate health concern (for example, new or worsening symptoms).

Please note that the evening clinic doctors do not prescribe pain medications or controlled substances, and do not complete forms (such as Special Diet, ODSP, Ontario Works etc.). Please make an appointment to see your family doctor/primary care provider for these reasons.
