Our work with Inner City Health Associates (ICHA)
ICFHT and Inner City Health Associates (ICHA) have a long-standing collaborative relationship providing comprehensive primary care to patients. An essential component of this care is the physician services offered through ICHA.
ICHA is a group of 200 physicians and nurses offering specialized services to people living on the street and in shelters, encampments and precarious housing across Toronto.
Through this partnership, ICFHT clients have access to primary care physicians, addiction medicine specialists, and psychiatrists, who work in close collaboration with our team of allied health professionals.
This collaboration extends to our work together at our outreach clinics, where we bring primary care to patients in shelters and drop-ins. Our aim is to assist patients with their health & psychosocial needs, and to help them transition to being cared for by a primary care physician in the community.
Together with ICHA, we are setting the standard of integrated care and support to the homeless community and are dedicated to addressing the social determinants of health. Through our collaboration we strive to improve both individual ICFHT client and population health outcomes.
