Who We Are

Inner City Family Health Team [ICFHT] is a small, community-based, not-for-profit Health Care organization located in downtown East Toronto focused on improving the health and well-being of people experiencing homelessness, particularly those whose complex health needs are shaped by systemic inequities and barriers to care.

Our multidisciplinary team of healthcare professionals offers comprehensive services in health, mental health, substance use, cognitive support, and healthcare outreach to mitigate the effects of homelessness and poverty. We prioritize collaboration with community agencies across our programs, which helps us expand our collective impact, enhance expertise, and increase accessibility.

Inner City Family Team members maintain a deep commitment to making healthcare inclusive and accessible, grounded in health equity, harm reduction principles and trauma-informed care.
